Has anyone had any issues using modifier AS for an APP as an assistant surgeon without using modifier 80? I saw some conflicting info on the forum due to a CMS document from 2008 stating "Modifier 80, 81 or 82 must also be billed when modifier AS is billed." I've never done this, I've always just used AS alone if it's an APP since the others indicate it was a physician. Just wondering if I'm in the wrong here. Thanks!

Thank you! Also, only one OP note is needed for a surgical assistant claim to be billed, correct? Co-surgeons is the one that needs separate notes.
That's correct. The OP note should document what tasks the assistant did and if a teaching hospital, a statement about why a resident wasn't used.
