i have noticed that when we submit our claims cpt 99204- with mod 25 and cpt 93000, its coming back as :

Medicare Florida denied for the CPT 99214 as Inclusive. claim rebilled with 24 & 25 Mod for 99214 CPT.

has anyone seen this, once we resubmit with mod 24 and 25, the claim gets paid. Is this something new?

We have had to do this with chemo patients who are coming for visits prior to chemo that are still within the global surgical period. The 24 for the surgery period, the 25 for the same day chemo. Haven't had a problem with the payers recognizing them used together.
