The procedure that was done is 36215, 36221,75710 26,59. It was paid in full and the insurance came and took there money back, but only paid a portion on 36221. Does 36215 or 36221 need a modifier?

Hello - I think you need one code or the other (36215 is a selective catheter placement and 36221 is a non-selective catheter placement), so that could be why they took their money back, usually you can't have both (selective and non-selective). It's also hard to know for sure without seeing the report.
