I am having rejected claims from UHC Medicare Advantage saying the JZ modifier submitted is inappropriate. I am not understanding what needs to be done. Anyone else having this issue? I would appreciate any help. Thank you.

Is the drug NDC a single use vial? If so then the JZ would be correct . If it’s not single use then the JZ would not be needed.
One other issue may be that you are putting the same drug/ same NDC on 2 lines. Insurance probably thinks it’s a duplicate. I would combine on 1 line for the drug if same NDC.
