It is my understanding that you don't need a 25 modifier on an E&M code or any modifier on any primary code when using add on codes. Is that correct? I can't find any documentation that specifically states that.

If I am reading your question correctly the answer is no modifier needed. So if you were billing 99291 & 99292 modifier 25 is not required on 99291.
