Modifier on OV with port draw??

All of our 36591's are denied as part of the OV. 90% of the time when the our patients have an OV they are also going to the lab and we end up charging an OV and a 36591. Should we be billing the OV with a 25 mod on it?

I believe there is something either in the CPT book or CCI edit regarding this. However I have always understood that you cannot bill an ov with a port flush

As far as i understood it, the port draw is considered inclusive with any other service. The only way you can bill for it is if that is all that was done.
