I have a Medicare patient who had an IOL with upgraded lens (Crystalens) which was supplied by the manufacturer at no charge to our facility, therefore we did not charge the patient, nor was an ABN signed. My question is would I code V2788 with modifier GL ?

I would not create a charge line for that item because it was given to your facility free of charge. Also on a different note the GL modifier is not an approved for ASCs. Hope this helps.
