Wiki Modifier QW

I have a question concerning the correct use of modifier QW....

I know that Medicare requires this modifier to be reported on all CLIA waived tests, but do you only report this modifier for Medicare or is it required for all CLIA waived tests regardless of carrier? Thanks.

I have always thought this was for Medicare only, unless your carrier specifically states to follow Medicare guidelines. I would check with your carrier to make sure.
