I have gotten cpt rejection for "procedure code 96360(hydration)requires an appropriate modifier when procedure 59160(dilation of cervical canal) present." CCI shows a modifier is allowed, but doesn't say required. I am at a loss as to what modifier would be appropriate and why one is even needed. Help please.
