If you have a patient that has to go back in for surgery the next day after they already had surgery. Would we use modifier 79 still on yesterday's surgery with our CPT code and then modifier 78 on today's surgery with our CPT code?

You would only put a modifier on the 2nd surgery. If it was a complication to the first procedure then use a 78. If it was a staged procedure (The doctor knew that he was going to do it the day before) then it would be a 58.
