E/M and 69209, what modifier should be used on 69209. Insurance is denying 59.

Did you run the combination through the CCI edits? 69209 is the Column 1 code. Any modifier would be need on the E/M service, if it's separately identifiable and supports the use of a modifier.

yes, did have modifier on the E/M. Plan is requesting on 69209 as well.

Unless there are other services billed that day by your office, there is no need for any other modifier except perhaps an RT or LT since 69209 is a unilateral code.
