Wiki Modifiers - billing a Barium Swallow

I keep getting mixed results from insurance comapnies when billing a Barium Swallow 74230, FLourscopy 76000 and lap band adj S2083. We have been putting the 59 mod on 76000 and not the adj. Can anyone give me some ideas?:confused
 
If the fluoro is done to adjust the lap band it is bundled with the s code. The last aapc coding magazine I got touched on lap band adjustments and indicated that imaging is included in the unlisted and s code for this. If the fluoro is done for or with the swallow it is also bundled with it and also should not be charged. Either way I don't think you should be charging for the fluoro.
