My issue is the same as everyone elses where I have appended modifiers to 92978 3 times and all 3 times the line was denied

No, it was one vessel 92978-LM. What I am saying is I have submitted this line to Medicare 3 times, modified different all 3 times and it was denied so I am not sure how to submit the 92978 to Medicare to get paid Thank you Nancy
