Wiki Modifiers on Add-On code 36907

Hi, I am new to this and could use some help. The hospital billed 36905 & 36907. Medicare is saying a modifier is needed on 36907. (OCE edit 40) Can some explain why we need a modifier on the add-on code when the CPT book indicates they can be paired together? Thank You
