I am getting rejections on all well checks 99391-99395 latley when they have a 90460 or a 90461 on the claim saying they need a modifier is the ep the correct modifier needed this has just started in the last month.
 
I apply EP modifier to CPT 99395 if it is Staywell MCD they required it for EPSDT visits (Early periodic screen diagnosis treatment) for age 18- 20
