Wiki Mohs Repair/Closure Diagnosis when done by provider other than Mohs surgeon

Does anyone have Mohs providers who refer to mid-level providers or plastic surgeons within their practice for the Mohs closure? I'm trying to determine what diagnosis the Mid-Level or Plastic surgeon should be coding.
  • When the provider performs both the Mohs and repair/closure the documentation/coding reflects the type of skin cancer.
  • When the patient is referred to the plastic surgeon or mid-level for the repair/closure the patient no longer has the cancer diagnosis as the purpose of the Mohs is to remove the skin cancer lesion in its entirety. Should the closing provider (Mid-Level or Plastic Surgeon) be documenting Open Wound without complication (or something similar)? or should the Mid-Level/Plastic surgeon be coding for the skin cancer?
Both Mohs and Repair are usually performed on the same day; however, there are occasions when the repair is done the next day. Any helpful information that can be provided would be greatly appreciated.
