Is it possible to put more that 4 ICD 9s on a claim form and link them to the CPT codes?
 
as of 2007 you may list 8 dx codes on the electronic submission but you may link only four of the eight to a single line item, any four but only four. As of 2012 you will be able to list 12 but link only 4
