I'm confused about how to bill 86317 for a 14 serotype lab test. The MUE for this code is 6, with a MAI indicator of 3 so with documentation to support medical necessity can override. But does that mean that the additional 8 units need a 59 modifier? Or do all the units above 1 need a 59 mod since they are all distinct serotypes tested? Or do none of them need a modifier with the documentation to support? I appreciate any opinions on this issue. FYI, working on auditing, I'm not a lab biller nor work at a lab.
 
I am wondering the same thing. 2020 CPT Assistant mentioned using the modifier 59 when separate results are reported, but does that mean we code one line of 86317 x 6, two lines of 86317 x 6 with modifier 59, and one line of 86317 x 5 with modifier 59?
