I have a patient that comes in for injection for both knees and his shoulder. I cannot figure out how to code the 20610's for all three to pay. I'm billing a BCBS Medicare plan.

I tried: 20610/50 1 unit for the bilateral knees
20610/LT 59 51 for the shoulder
 
I don't bill 20610 but I would think that since the MUE for 20610 is only 2, you would need to appeal any overages billed over 2 units.
 
I wouldn't use the 51 with the 59 on the shoulder. I would just make sure my diagnoses line up with my injections. For example 20610-50 dx M17.0, 20610-59-LT, dx M25.512. I agree, you may have to appeal for over 2 units.
