Is it acceptable to use a 25 modifier more than once in a single office visit?? The exact senerio in question is this: A pediatric well check (99393) with vaccines (90460, 90732), and a seperate e/m charge for a.d.d medication check.(99213). Does the 25 modifier apply to both the 99393 & the 99213 in this case or is it appropiate to use it 1x on the 99213?
