Physician provided HPI and partial exam on 3/16 & finalized exam and MDM on 3/17. Should we bill two separate visits on the two separate dates OR bill one visit on the last date, combining the two?

Would you base this visit on time if counseling was documented as >1/2 of the visit even on two dates? Thanks!
 
Hmmmm

My inclination is to code only for day 2 and include the HPI and elements of exam performed on day 1 as part of the entire consult.

BUT ... I'd feel more confident giving my opinion after seeing the documentation. I'd sure like to know why the entire visit was handled in one day. What were the unusual circumstances that resulted in this scenario?
