If an NP in a multispecialty office does an exam, provides some infusions and then also has a PT provide some therapy services on the same day, can both the NP and PT put charges on the same HFCA form with each charge line showing the rendering provider's NPI, and then the NP signing in box 31 and the group practice name and group NPI in box 33 or do the two providers need to be on separate HCFA forms? And what if the other provider is not a PT but is a chiro or other physician?
 
As long as all of the providers are under the same group NPI and TIN, and services were rendered at the same location, you can bill them on the same form. The individual NPI of each provider goes in box 24J to identify which provider performed which service.
