Multiple I&D Intraoral + Teeth Extraction

I am new to OMFS Coding. When I have more than one Intraoral/extraoral I&Ds performed within the same service, do I code every site separate or just one? Also, it is my understanding that you do not code the teeth extractions in the same location as an I&D, is this correct as well?
