My doctor performed laminectomies for decompression on levels L2-3 and L5-1 and laminectomies for removal of epdidural synovial cyst and adhesive thickened ligamentum flavum on levels L3-5, L4-5.
I propose coding as follows:
63267 for L3-5
63047-59 for L2-3
63048 for L5-S1
However, this is a Medicare patient so no operative report will be sent. How do I indicate that the procedures were on different levels?
