Hello, If a provider documents 3 procedures on one record with all the required documentation for each, are they billable? Can he create one single progress note in which he documents each procedure he performs? For example, he performs a central line, intubation, and arterial line and places the documentation of all three on one procedure/progress note. Will this meet documentation requirements or does each procedure need to be documented and saved on a separate progress note?
