1.)Can I bill 92014 WITH 92015 AND 92226?
2.)And it is necessary to attached a MODIFIER (LT/RT OR 50) fo 99225 and 99226?

92015 is a refraction and not a covered service by Medicare the GY modifier would be attached to this code. 92226 a subsequent ophthalmoscopy will require which eye or both modifiers. In addition, a 25 modifier will be attached to the visit.
