Hope you can help. One practitioner saw pt on last day in hospital, billed a f/u code, not a discharge code. Another practitioner saw same patient, same day, upon his return to SNF. Billed as an initial visit. I can only find rules for D/C code at hosp, followed by SNF initial. What is the rule for above? Thanks!
