If the documentation states "Normal range of motion muscle strength, and stability in all extremities with no pain on inspection", should this be counted as a complete musculo exam? It is my understanding that each extremity that is examined should be documented separately in order to get credit.

I would say no. For the detail that would document a complete musculoskeletal exam, you should look at the 1997 Coding Guidelines.
