Wiki MV Repair then MV Replacement ? Modifier

My surgeon did a mitral valve repair with ring (33426). After adequate rewarming and weaning from cardiopulmonary bypass a TEE was done which showed pt to still have a central jet in the area of the A1-AP2 junction. He felt this was too much regurgitation to be satisfied with the repair so he placed the patient back on bypass and removed the ring and did a mitral valve replacement with a #29 Carpentier-Edwards pericardial valve.

Is the correct way to code this procedure to just code 33430 with a 22 modifier and send the op note??? Having a "senior moment".....

I actually think I would bill for both procedures, 33426 and 33430, with possibly a modifier 52 appended to the 33426.

The surgeon did perform both procedures and as long as both are dictated accurately, you should have no problem billing for both.
