Wiki N18.30 vs N18.31, vs N18.32

I am having an issue with a 3rd party vendor who is stating that if a provider codes N18.31 or N18.32 with MEAT, but uses the term CKD3 only in the discussion of care that the code needs to be N18.30. Thoughts?

If in the A/P it states CKD3 and there is MEAT for it, then N18.30 would be appropriate. Just because the provider chose the code N18.31 or N18.32 doesn't mean that level of CKD3 is supported. The provider will have to document CKD stage 3a or 3b in the note to be supported.
