It's pretty self-explanatory if you're using the alphabetic index to look up the codes.

Alphabetic Index:

Nausea (without vomiting) R11.0

sls314 is right about the index clarifying the titles.

Additionally, there is a guideline telling us that if a combination codes exist, always use the one combination code instead of the two codes that equate to the combination code. This is helpful when insurance requires your medical necessity be in the top 6 codes - you don't want to waste slots by using the two codes when the combination code exists.

From the guidelines:
"Assign only the combination code when that code fully identifies the diagnostic
conditions involved or when the Alphabetic Index so directs. Multiple coding should
not be used when the classification provides a combination code that clearly identifies all of the elements documented in the diagnosis. When the combination code lacks necessary specificity in describing the manifestation or complication, an additional code should be used as a secondary code."
