Can someone advise on the correct way to enter NDC codes? Do all commercial and MA insurance require the 11 digit code? Carefirst and Tricare are rejecting some claims but paying majority of them. There doesn't seem to be a rhyme or reason to their denials. Any help would be appreciated.
