I have a product that is listed on CMS NDC-HCPCS Crosswalk with a NCD code that is 12 digits long. My clearing house is rejecting the claim stating the NDC code must be 11 digits. I've received absolutely zero help from the products representative and billing assistance department. Does anyone know the trick for converting from 12 digits to 11 digits?

I have never encountered a drug with a 12-digit NDC number - can you share what the drug is or provide a copy of the package that shows this?
