Please help!!! I am having difficulty getting paid for j3301, j0702, s0020 from United Health Care. I know that a line note is required. If 4 units of J3301 is use, I believe the line note should include the ndc # in the format N4xxxxxxxxxxx (11 digits) I'm not sure what else It is 400 mg per 10 ml. Thank you
