NDC's on J-codes on claims

I know most (if not all) insurances require the NDC's (as well as units of mearsurements and NDC units) to be attached to the J-codes on the claims. Is anyone else finding this a challenge since NDC's change so often?
 
The only time the NDC should change is if you purchase the drug from a different manufacturer or in a different size. Is this happening that often?
 
Yes, it does happy pretty frequently. The challenge mostly lies with entering the NDC's period though since each patient has so many J-code's on any given encounter. I was just wondering if everyone else enters the NDC's for each J-code.
