Hi,

We are getting denials from BCBS with CO107 remit code when cpt 94640 billed alone on a facility claim, does anyone has the same issue we have?
 
I am following this thread as I bill for professional services but our facility billers just came to me asking about 94640 being denied as not covered when billed with an ER level.
