Wiki need an opinion to differentiate 99213-99214

I really need a second opinion on a scenario.

Coumadin visit plus a DX for leg pain that was diagnosed prior to this visit. What would you use? 99213 or a 99214??

Hypertension follow up not stated as uncontrolled..99213 or 99214? If lower let me know.

it depends on if the issues are improved/stable/worsening.... medication management automatically gets a moderate for the MDM, but what about the exam? what level did the dr document? Also what about the history? (HPI elements brief or extended? and ROS, how many were charted? and PFSH, how many?) All of this levels the E/M code. I would download an E/M audit worksheet and use this for leveling.
