I am coding for a psychologist who is seeing a teen with diabetes. The teen is noncompliant with his insulin but is no depression or Anxiety. Not a new DM patient so it's not an adjustment disorder. Thoughts? Currently using Z91.19: Patient's noncompliance with other medical treatment and regimen but it's not payable.
 
If the patient is underdosing his insulin, then you need a code such as diabetes with hyperglycemia followed by he Tcode for insulin use with the sixth character of 6 and the 7th character A D or S, the the Z 91 code third for noncompliance.
 
But the provider is psychologist so she's not controlling his insulin or diabetes more evaluating him as to why he is noncompliant. If I code an "e code" it's not payable.
 
You will need to still code the underdosing first and underdosing T codes must be secondary to the consequence of the underdosing. The z91 codes are not to be used first listed. This would be the issue with why the claims deny. There are no "E" codes in this scenario.
