Can someone help me with the coding of the following report?
"Indication: Chronic severe back pain, filled steroid injections for pain management, nonoperative candidate.
Procedure: The patient was informed and consented for the procedure the prepped and draped in the usual sterile fashion. 1% lidocaine without epinephrine was utilized as a local anesthetic. Under fluorscopic guidance, percutaneous access needles were placed from a posterior lateral approach into the doral central aspects of the left and right L4-5 and L5-S1 facet joints. Access needle positions were confirmed with sensory and motor nerve testing at all four sites. Bilateral L4-5 and L5-S1 facet/median nerve root of radiofrequency ablation was performed at 80 degrees celsius for 90 seconds at each site. No complications were encountered.
Impression:
Successful fluorscopic guided L4-5 and L5-S1 bilateral facets/median nerve radiogrequency ablations."
