Wiki Need cpt code for Revision Basilar Joint Arthoplasty

Provider stating what he really did was revision LRTI as the trapezium was already gone. He's thinking cpt 25445 with mod 22 to account for the additional work. Pt had a lot of scar tissue and he did a lot of extra work due it failing previously.

Can you post the redacted op note? Otherwise we are shooting in the dark here. Much of 25447 is performing the carpectomy which was previously performed.
 
Agree, it's tough to say without seeing exactly what was done. There is no CPT to revise a previously performed 25447. It will depend on what was done. It is possible it could even be more of ligament or tendon transfer type CPT.
If there was no implant/prosthesis and trapezium work it's not 25445. If anything and repeat LRTI it would just be 25447 again (depending on what was done).
