Hi,

I am new coder for wound care, my question is patient was coming from SNF to acte care inpatient hospital for wound care, Dr. A had been managing wound care for 4 days and then 5th day transferred to SNF facility(same hospital). could you please someone help me shall I use 99221 -99223 for initial day admit with source of admit code "D" & 99231-99233 for 5th day transfer with discharge status code 03?

Please advice me if am I wrong?
