Recently I encountered a scenario, patient presents with migraine follow up, Explains for past 1 week have been getting more frequent and after consuming medication experiences numbness in the extremities and dizziness and physician wants to stop the old RX and trial new. In this scenario which table A point we need to consider either 1 chronic illness or 1 chronic illness exacerbation.
 
Level 3 for one stable chronic illness - key word is stable. If they are not at goal or experiencing an exacerbation, side effect, or progression of the disease then level 4 is appropriate for column 1
