Need help billing.. New patient came in office visit took 20mins. and he has pain in both left M79.674 and right great toes M79.675, B35.1 Onychomycosis... what is it that I'm missing? A modifier? they're denying buz a status code is missing ...what one? Please help... I'm sure it very simple just need help
