Our physician performed a cervical dorsal medial branch radiofrequency bilateral C4, C5. What CPT code would I use? I appreciate any help I can get.

Just to clarify, depending on the DOS of the procedure...64626 is no longer valid, effective 1-1-12. May want to look at 64633-64636 if it is after 1-1-12 :)
