our doctor found out that patient doesnot qualify for icd implant after he inserted icd and leads so he explanted both leads and icd . how should I code? 33249, 33241-78,33244?
 
If it was inserted due to a physician mistake, i don't think you can bill for it at all. Wouldn't this be something that should be determined before scheduling the surgery?
