Patient comes in and has a pink pearly papule and Dr wants to biopsy it. Can i bill a 99214 1( new issue with uncertain prognosis/decision to biopsy) AND biopsy code? OR with the way new guidelines read would you only bill for the biopsy UNLESS you bring patient back on different day????

would i need another "problem" with the pink papule in order to bill the E/M code with biopsy??
