Hey Guys I know this really doesn't need a new forum but I wanted to know if someone can help me find the LCD for Illinois. I tried the Medicare website and I found no luck. If someone would like to attach a link or even a PDF file I would very much appreciate it. Also I am trying to call on claims to have a live person tell me whats wrong with it. The machine doesn't tell me much. Can someone please help me with these two things.

Look at your Medicare remit (EOB), there is a remittance advice remark code,
Ex.
MA61 Missing/incomplete/invalid social security number or health insurance claim number.
There will also be an adjustment code,
Ex.
PR-31 Patient cannot be identified as our insured.

These codes will tell you why the claim has rejected. Medicare customer service reps WILL NOT tell you why a claim has rejected. They will tell you to refer to your remittance advice (EOB).
