Dear All,

I am struggling to find CPT codes for below procedures. Please can someone help me in getting the codes.

1) Radiofrequency Neurotomy of l4-l5, can i use this codes - 64622 and 64623

2) Discogel Injection to l4-l5 - 62290 .
