I code for an APN and need help with the following visit.

Patient came in to have a urinary cath removed that was placed by in the ER several days earlier. APN removed the cath but later that afternoon the patient came back saying she was unable to urinate so a foley cath was then placed. How would I code that visit?


Also would I bill for the removal of the foley cath once we remove?

THIS IS A MEDICARE PATIENT.

The cath removal would be included in whatever level e/m code is indicated by the documentation. Since the patient came in later, you could append modifier 25 to the e/m and bill out 51702. Since I don't know what was documented, I can't really help you with the e/m code, but there is no specfic code for cath removal.
