I asked a question yesterday about the modifier 25 being used with and EM code and an urinalysis - 81003. Our billing department is stating this needs to be used with every 81003 when (as coders) bill this out. I work in Ohio and I feel this is an over use of the modifier. If anyone can help with documenation in writing it would be greatly appreciated.

We don't use modifier 25 on any E&M when billing lab too. We never have a problem getting paid without the modifier. However, if it's Medicare, certain labs do require the CLIA number and QW modifier. Could that be part of the problem?
